Located in the Village of Mount Horeb, we are a short 15 minute drive from the West side of Madison.
We currently have openings for a part-time or full-time Registered Nurse (RN) or Licensed Practical Nurse (LPN) to work PM and/or NOC shifts.
The Nurses we are seeking have proven strong clinical and leadership skills, and make person-centered care a priority.

Experience:
Graduate of accredited school of nursing Current state licensure as a Registered Nurse or Licensed Practical Nurse in good standing CPR certified Must be English literate and possess excellent written and verbal communication skills Long-Term Care clinical experience as a Registered Nurse or Licensed Practical Nurse (preferred) Essential Functions of the Job:
Knowledge of (geriatric, long-term, rehabilitation, and/or acute medical care) nursing principles and professional standards of nursing practice and ability to apply to resident specific circumstances.
Able to identify, implement and evaluate appropriate objectives and interventions for residents.
Demonstrated competency in clinical skills needed for facility resident population.
Demonstrated competency in physical, behavioral and functional capacity assessment of long-term care residents.
Working knowledge of laws and regulations that influence provision of care and services in nursing facilities.
Working knowledge and ability to interpret and demonstrate facility policies, procedures to direct care staff.
Sufficient organizational and interpersonal skills to assign, motivate staff, elicit work output, improve quality and interact effectively with residents, families, interdisciplinary team members, administration, government officials, consultants and others in addition to staff.
Demonstrates knowledge of medications and their correct administration based on age of the patient/resident and their clinical condition.
Evaluates patient/resident responses to medications.
Observant for significant changes in resident's condition and communicates this to the physician and IDT for the purpose of the MDS.
Demonstrates and is knowledgeable of the importance of maintaining confidentiality of resident and facility information.
Plans for and provides patient/resident, family and caregiver education, identifies learning needs, clearly communicates information and documents patient/resident response to education.
Must have organizational skills, ability to assess, plan and implement and evaluate patient/resident care.
Must be able to demonstrate ability to handle multiple priorities and the ability to direct and evaluate the activities of others.
