He/she coordinates admission to the appropriate program with the team leaders.

Summary:
The Home Health Intake Coordinator is responsible for each step of the intake process of our home health referrals.
In this role, you will serve as the first point of contact for discharge planners, physicians, and agency staff and be responsible for sourcing and collecting patient information and distributing it to the appropriate team members; maintaining patient files and setting up charts; and coordinating with nurses and therapists to verify that appropriate services are provided in a timely manner.
The Home Health Intake Coordinator will also have general administrative duties such as answering phones and filing paperwork as needed.
The Intake Coordinator works under the supervision of the RN Clinical Manager and Administrator.
The primary functions of this position are to respond to inquiries about existing services and other resources in the community, initiate and facilitate the referral intake process for patients in all stages of life, facilitate the flow of patient information to/from staff nurses, patients and physicians, and to assist staff with problem solving.
Requirements:
Be committed to and passionate about 100% customer satisfaction High school diploma or the equivalent Have at least one year of experience in home health Demonstrated experience using Microsoft Office, including Word and Excel Excellent verbal and written communication skills have a genuine interest in caring for patients.
Be able to maintain patient and tactful composure when dealing with residents, family members, staff members and visitors Manage EMR Enters staff and client information into database Assisting with nursing needs if they call in and need something, faxed, mailed, etc.
Maintain patient hard charts as well as patient chart in Matrix Assist in Scheduling Patients Coordinate with Patient, Physician, Caregiver, Clinician and Management effectively Manage Medical record Manage office resources when needed Assist Administrator/nursing/therapists as needed Job Type:
